Based only on my personal experience, are Carl Zambuto, who no longer sells to individuals, and Richard Wessling, of The Pines Optical Shop, in Milford Ohio, who does sell to individuals.
Wessling does all the work himself, so your mirror gets individual attention. He has made large and small Newtonians, and also Classical Cassegrains, Herschelians, Shiefspieglers, Tri-Schiefspieglers, Yolos, and many unusual and demanding designs, some of them using toric and other aspeheric mirrors. A truly outstanding mirror maker.
I have a Zambuto (in an 11 Starmaster) and two Wessling mirrors, 10 and 6 inches, and all three are outstanding.

Steve Swayze has been mentioned and his are all made with his high standards of perfection. Carl Zambuto has a fine reputation, but as has been mentioned, he does not sell to ATM.
Mike Spooner has not been mentioned, but his work is held in high regard, making some of the best Planetary mirror you could find any where.
John Hall makes the same high quality mirrors and his reputation speaks for him.
There are many more that also make fine mirrors. You have to decide what f-ratio you want and how soon you want the mirror as some of these men take a while to get one finished.
But I would take any one of the mirrors that these gentlemen made and be very pleased to own it.
