TAMRON A03 AF28-200MM F/3.8-5.6 XR Super Zoom Lens for Nikon.
Data from Tamron:

The Nikon A03 AF28-200MM F/3.8-5.6 XR takes the prize as the world's smallest, lightest 28-200mm high magnification Zoom lens. With a minimum focal distance over the entire zoom area of 49cm, and a maximum magnification of 1:4 (at 200mm), this remarkable lens achieves high magnification zoom performance, while maintaining the compact size of a standard zoom lens. Use of XR (Extra Refractive Index) glass and efficient use of aspherical lenses helped achieve marked reductions in size and Weight over previous models, without compromising image quality
